What is CO2 Laser Skin Resurfacing?

It is a type of facial revitalization procedure which uses a laser to improve the skin texture and appearance. This procedure involves the precise removal of skin layer by layer. During the healing process, the new skin appears tighter, with a younger-looking surface. It can be done with:

  • Ablative laser: where the laser removes the outer layer of skin (epidermis) and heats the underlying skin (dermis) with the growth of new collagen fibers. As the healing process ensues, the treated area appears smoother and tighter. This type of treatment includes carbon dioxide (CO2), erbium laser, and a combination system.
  • Non-ablative laser: which stimulates the growth of collagen, which over time helps to improve skin tone and texture. This approach can be done with intense pulsed light (IPL) devices. Non-ablative laser treatment is less effective than ablative laser.

How does the CO2 laser resurfacing work?

As already mentioned, laser skin resurfacing targets the outer layer of skin with simultaneous heating of the underlying layer. This promotes collagen production which helps produce new skin that is smoother in texture and firm to touch.

The newest version of CO2 laser resurfacing (fractionated CO2) uses very short pulsed light energy (Ultrapulse) or continuous light beams that are delivered in a scanning pattern.

Who is a good candidate for this procedure?

Laser skin resurfacing can be used to treat one or more of the following skin conditions:

  • Acne scars
  • Age spots
  • Wrinkles or fine lines
  • Skin sagging
  • Scars
  • warts
  • Uneven skin tone
  • Crow’s feet (little lines spread out from the corners of your eyes) This procedure is not suitable for people with active acne breakouts, stretch marks, or excessive skin sagging. It is recommended that this procedure is carried out during the fall or winter season to help decrease sun exposure which can damage the delicate skin.

What to expect?

It is an Outpatient procedure (no overnight stay). Before the resurfacing procedure, the skin is prepared with a series of treatments done several weeks before the procedure. This is done to minimize the side-effects of the procedure and to increase the skin’s tolerance to the treatments.

The doctor will advise you to avoid taking any medications or supplements like Vitamin E, Aspirin, or Ibuprofen which can affect the clotting process. Smoking should be avoided for 2 weeks before and after the procedure as it prolongs healing. Antibiotics are also started to prevent bacterial infections and antivirals if you are prone to cold sores or fever blisters.

On the day of the procedure, the laser therapist will apply a topical anesthetic to the area to be treated. This is done to decrease pain and to make you more comfortable during the procedure. If a large area of skin needs to be treated, the doctor may give you a sedative or painkillers.

This is followed by cleaning the skin to remove any excess oil, dirt, and bacteria. The CO2 laser is moved slowly over the area of the skin to be treated. The procedure takes about 30-45 minutes if only a part of the face is to be treated.

Finally, dressing of the treated area is done to protect the skin. After 24 hours of treatment, the treated area should be cleaned 4 – 5 times a day followed by the application of petroleum jelly ointment to prevent the formation of scabs. Healing usually takes 10 – 21 days, depending upon the condition treated.

Aftercare

A broad-spectrum sunscreen that has a physical blocker, such as zinc oxide, and a sun protection factor (SPF) of 50 or higher should be used. Sunlight exposure should be limited, especially between 10 am to 2 pm. Also, a broad-brimmed hat to protect the skin from harmful sun rays should be used.

Moisturizer should be used about 6 weeks after the procedure or when the doctor advises. Once healing has occurred, make-up can be done to hide the pink to red color of the procedure. It takes around 2-6 months for these to disappear. Oil-based make-ups should be avoided.

A dark-skinned person is more likely to get darker pigmentation. This can be minimized by using a bleaching agent before and after the procedure.

Are there any complications of the procedure?

Like any other cosmetic procedures, laser skin resurfacing also may have some complications like:

  • Burning
  • Rashes
  • Redness
  • Swelling
  • Hyperpigmentation
  • Reactivating herpes cold sores
  • Bacterial infections
  • Scarring

It is normal to have swelling following the procedure. Sleeping on an extra pillow at night to elevate your head and putting an ice pack on the treated area can help reduce swelling in the first 24 to 48 hours.
